Connections is a unique program that bridges gaps in care, and empowers and supports community members as they navigate the network of behavioral health services in Larimer County. We have a diverse group of trained behavioral health providers who can offer referrals and care coordination to help adults find care that addresses their unique concerns and goals. In addition, we offer brief therapy and intensive care coordination to those who experience barriers to care.
