Senior Reach is a community program that identifies older adults who may need in-home emotional or physical support and/or connection to community services. This service is still available and has moved to a telehealth model. Seniors can call and learn more, or someone can call and refer a senior (doctor, loved one, etc.). This service focuses on two aspects of senior care: therapy and care coordination. Call for more information.
